Requests for the release of information pertaining to the Family Violence Information Sharing Scheme or Child Information Sharing Scheme must be completed by an Information Sharing Entity (ISE) and/or Risk Assessment Entity (RAE).

Our Information Release Service will arrange for an Eastern Health authorised person to contact you and facilitate the request.
