Ensure your request contains the following to enable our service to identify the records or information you are seeking. This may include:
- Full name of the individual;
- Individuals date of birth;
- Date of attendance or admission;
- Site or service attended;
- Type of document being sought; and
- If applicable, what legislation is being relied upon in making the request.
If you are not clear enough, staff will contact you to clarify the scope of your request, but this may result in processing delays.
