The Eastern Health Integrated Renal Service (EHIRS) was established in 2008 to service the growing health needs of people living in the eastern suburbs of Melbourne.
The service provides a comprehensive care for people with kidney disease, including:
- Management of acute kidney disease in hospital, including dialysis and plasma exchange when required
- Outpatient management of acute and chronic kidney diseases
- Preparation for dialysis for those who require it, including provision of information and education
- Surgery and radiological procedures to enable people to have dialysis (“dialysis access”)
- Provision of chronic dialysis, including haemodialysis at home, haemodialysis in a Satellite Unit, and peritoneal dialysis
- Assessment and preparation for kidney transplant
- Care of kidney transplant recipients after discharge from hospital (the surgery is performed at another hospital)
- Allied Health services including dietetics, social work and psychologist support
- Research into kidney disease and its complications
We encourage and support dialysis at home where appropriate and have strong links with community care providers to help manage those patients for whom dialysis is not appropriate.
In addition, we provide both inpatient and outpatient Obstetric Medicine services to pregnant women with pre-existing or new medical conditions receiving Obstetric care at Eastern Health. This is a collaborative service involving Physicians, Obstetricians and Midwives in a multidisciplinary model of care that includes consultation with other medical subspecialties where required.

Patients will be triaged according to their presenting problem and where possible, offered an appointment in a clinic location closest to their home address (subject to appointment availability and waiting times).

At Eastern Health, we accept referrals to Renal Medicine for the following as per clinical assessment and need:
- Pre-dialysis, dialysis care
- Kidney transplant recipient assessment and post-transplant care
- Live kidney donor assessment and post-donation care
- Acute kidney diseases including glomerulonephritis
- Renal hypertension
- Renal dialysis patients
- Pregnant women who have specific renal medical issues
- We prefer to see women with medical conditions prior to conception where possible, to allow time to optimise their management for pregnancy. However, we manage new and old medical issues and see women in the post-partum period until medical stability, before discharging patients to their previous model of care.

There is a Renal Registrar on-call from 8am Monday to 12pm Saturday every week, and there is always a Renal Consultant on-call. This will facilitate admission to the ward or early outpatient review as appropriate.
Good quality information will ensure the referral is appropriately triaged for a timely review in the most appropriate clinic. Please include a statement as to your primary concern, the serum creatinine and other results (MSU, urine protein: creatinine ratio and renal ultrasound). If an interpreter is required, please indicate this as well as the language required.

If your patient’s condition deteriorates, please send an updated referral via HealthLink SmartForms selecting the Referral Continuation “Amended referral/update previously sent referral” with additional clinical information.
