The Eastern Health Primary Mental Health Team is part of the public mental health service system and provides among other things an outreach model of mental health shared care to primary health care providers (PCP) located in the local government areas of Manningham, Monash, Maroondah, Whitehorse and the Yarra Ranges.

The Eastern Health Primary Mental Health Team (PMHT) is a multi disciplinary team which aims to support the primary care provider groups of General Practitioners, Community Health Services, Psychiatric Disability Rehabilitation Support Services, Maternal Child Health Nurses and Family Services across the Eastern Metropolitan Region.

The teams provides primary consultations (direct care) via biopsychosocial assessment and collaborative shared care planning for primary care consumers of any age who have mental health problems such as anxiety, depression or related disorders, including co-occurring disorders, which have an impact on their quality of life and wellbeing.

This may include assessment for diagnostic clarification of anxiety and depressive disorders, provision of a range of short term biopsychosocial treatment interventions, and advice regarding referral options.

Additionally PMHT provides, secondary consultation (telephone and face to face) and tertiary consultation and education and training, including health promotion on a range of mental health issues.

Training and education can be provided on a one to one basis or within group settings.

Benefits of using the Primary Mental Health Team include:

  • Access to a multidisciplinary team including consultant psychiatrist.
  • Assisting the person to identify the main issues for mental health interventions.
  • Assisting people to identify and access other support services within the community, including assistance with navigation of the mental health system.
  • Review assessments where there has been an incomplete response to other interventions including assistance in the development /management of treatment plans.

Please note that the Primary Mental Health Team is not a crisis service and does not have the capacity to provide a service to those people with acute risk or currently in crisis.

Presentations for problems related to psychosis, hypomania/mania and acute risk should be referred to the Psychiatric Triage Service on Ph: 1300 721 927.

Faxed referrals will be initially acknowledged by the PMHT fax back form with a follow up telephone call.

Referral address is that of the primary care provider.

Upon acceptance of the referral the PMHT meets with the referrer and the client at the Primary Care Provider’s rooms and there is no cost for the service. For all face to face consultations it is anticipated that the Primary Care Provider is willing to work within the Primary Mental Health Team shared care model.

PMHT utilises a shared care model as the underpinning element of a collaborative approach to care. The intent is to support the individual and the individual’s primary care provider in accessing mental health interventions via the provision of a comprehensive approach to mental health care within a familiar and non-stigmatising environment.
