Eastern Health has an Ophthalmology Service which is based at Eastern Health Lilydale. The service provides care to patients in the Eastern Metropolitan Region of Melbourne.
The Ophthalmology service will only accept eReferrals (HealthLink SmartForms) for practices with conformant software (Best Practice, Genie, Medical Director and MedTech Evolution, Zedmed, and Shexie).
Where referrers do not have conformant software (e.g. Helix), referrals can be made via the MyHealthLink Portal. To register for a ‘free’ HealthLink Portal licence, please complete the online application form and select the two checkboxes – Receive Electronic Correspondence (Free ) and Smart Forms eReferrals (Free).
Referrals will no longer be accepted via post, fax or email.
Although the service is primarily aimed at patients requiring Cataract Surgery, we do also provide treatment for a range of conditions, such as :
Chalazion, Pterygium, Entropion, Ectropion, Laser Peripheral Iridotomy , Laser Posterior Capsulotomy ( for lens opacity).
We welcome referrals from practitioners in the Eastern Metropolitan community. Referrals needs to be made by e-referrals
Referrals can be made by:
- Optometrists
- Ophthalmologists
- GPs (must have Optometry report if a referral is from GP)
Information is available on HealthPathways Melbourne for assistance in managing your patient’s condition. This includes what investigations you need to perform prior to referring to a specialist and what information you need to include in the referral. Referrals for the below condition should meet the Statewide Referral Criteria for Specialist Clinics:
- Assessment for cataract surgery
For all other Ophthalmology service conditions listed below, please continue to refer as per clinical assessment and need:
- Pterygium
- Eyelid disorders (including entropion, ectropion, ptosis etc)
- Chalazion
- Posterior capsule opacification (PCO) requiring YAG laser capsulotomy
- Narrow angle glaucoma risk requiring prophylactic YAG laser iridotomy
Referrals for patients from the ATSI community will be fast-tracked in the interests of Closing the Gap.
No paediatric service.
Referrals to this service are assessed against the Statewide Referral Criteria and will be declined if the following criteria is not included within the referral:
- Patient’s name and date of birth and at least one valid phone number
- Copy of report from Optometrist or Ophthalmologist with best corrected visual acuity
- Brief medical history – comorbidities, medications, Weight less than 130kg and BMI< 40
- Reason for referral
- Service required

If your patient’s condition deteriorates, please send an updated referral via HealthLink SmartForms selecting the Referral Continuation “Amended referral/update previously sent referral” with additional clinical information.
Emergency cases should be sent to the Emergency Department.
