Eastern@Home incorporates three programs:
Hospital in the Home (HITH)
• Residential Inreach (RIR)
• Yarra Valley District Nursing Service (YVDNS)

Eastern Health’s Hospital-in-the-Home (HITH) service is one of the largest services in Victoria. Medical and nursing staff provide care in patients’ homes, at our HITH clinic or residential aged care facilities.

Patients admitted to the HITH program receive 24 hour, 7-days a week care and support, either on-site or via telephone support. Patients receive an initial medical assessment at our HITH Clinic in 18 Grey Street, Ringwood East (next to Maroondah Hospital) following acceptance into our program. Eastern Health is medically accountable for the patient until discharge from our service.

Services HITH offer include IV antibiotics for patients with cellulitis not improving after 48-72 hours of oral therapy, urosepsis with multi-resistant infections, wound management, IV infusions, post-surgical management of drain tubes, Iron infusion service and oncology treatments. Our service is beneficial for people in residential aged care facilities who can receive care in convenient, familiar surroundings.

The Residential Inreach Service provides an alternative to emergency department presentations for clients in residential aged care facilities (RACF’s). This service aims to support clients and staff to manage acute health issues when GP or locum is unavailable.

Referrals are triaged and followed up by specialist nursing/medical staff as required.

We can provide:
• Medical assessments of acute conditions
• Phone support and advice to facility staff about clinical decisions(our nurse can visit if required)

Acute Conditions can include:
• Chest infection/ UTI/ Cellulitis
• Gastroenteritis
• PEG and urinary catheter trouble shooting (not routine changes)
• Wound complications e.g. infection
• End of Life care and support to facility staff in care management

Yarra Valley District nurses provide nursing care to people in their own homes, every day of the year.

Our District Nursing Service is supported by funding from the Commonwealth Home Support Program, Department of Veterans Affairs and Victorian Governments including Home And Community Care Young Persons Program and Post Acute Care.

Examples of care that can be provided:
• People who need care immediately following their discharge from hospital
• End of Life and /palliative care
• People of any age who have a disability and who need nursing assistance to manage their healthcare needs
• Wound Care
• Diabetes management
• Continence management
• Medication management

Helping you find and link with other community services for older people.
