Our Healthy Living and Diabetes program provides early intervention for those with newly diagnosed Type 2 Diabetes and Prediabetes. Clients are provided with information about diabetes, and are encouraged to set achievable goals and make healthy decisions to improve their lifestyle.

A key worker will keep in touch with the client and their GP for a year to provide support. Our staff are trained in chronic disease self management techniques and work together as a team to provide a comprehensive service. Exercise programs are available.

Fees are $10 for low and medium income.

Groups run on Tuesday and Thursday.

Clients can see a diabetes nurse educator, a dietitian, a podiatrist, a physiotherapist, a counsellor or a Quit Smoking nurse. Other allied health and support services are also available. There are also exercise groups.

Our Service Coordinators will accept referral from GPs or other health professionals, as well as self referrals. See contact details.

Anyone who has been newly diagnosed with Type 2 diabetes or with prediabetes can attend. Services are subsidised for low and medium income.

Clients can learn about diabetes, healthy eating and cooking, exercising, foot care, setting goals and improving lifestyle. We can help you to lose weight and control your diabetes.

The initial group education day is $8.00. Individual visits to service providers will be charged according to the Community Health Fee Policy i.e. $10.00 for low and medium income.
