The role of Community Rehabilitation Programs is to optimise a person’s functional ability and independence.

Community Rehabilitation offers short term goal directed rehabilitation aimed at improving client’s function and independence following a change in function due to recent acute medical / health event.

This is achieved by developing a care plan based on goals that are negotiated between the client and their team (see below for team details).

Therapy can either be delivered in a group or individual setting. Clients can be seen in their own home or in one of our centres, dependent on their needs and goals.

• Client lives within Eastern Health catchment or is able to attend one of the centers for therapy
• Client consents & is able to participate in rehabilitation
• Client has experienced a change in function/activity limitation due to recent acute medical/health event
• Client is medically stable, able to be managed in the community by their GP (nursing care is not available in this program)
• Client has functional rehabilitation goals that are short term & realistic
• Client is over 18 years of age

Our service may not be provided if:

  • Risks to staff are not satisfactorily managed (behaviors of concern, environmental hazards, violence/aggression, history of drug /alcohol use of client/family/support persons)
  • Referral is for major home modifications or complex equipment prescription – please redirect referral to Community Health
  • Referral is for vocational rehabilitation
  • Referral is primarily for chronic disease management – please redirect referral to HARP
  • Alternative providers are available (Workcover, TAC, NDIS, Home Care Package)

Community Rehabilitation does not provide primary rehabilitation support for mental health, drug or alcohol dependencies

Doctor or allied health referral preferred, however self-referral accepted

In your referral, please include:

  • Client details including address, date of birth and contact phone numbers, including NOK
  • Reason for referral, including goals of therapy
  • Relevant clinical history for the client (include any relevant pathology/radiology reports)
  • Any risks to clients or staff

If information is not made available that may impact on our ability to provide safe & effective care we will request this of you which may impact the commencement of therapy for the client.

There is a cost for our service, which we will explain to you when we call to discuss your treatment plan.

We understand that covering the cost of health can be hard, even if you received the service in a public hospital. If you are worried that you may not be able to pay, please contact Patient Revenue Services to talk through your situation.

Frequently Asked Questions

Yes, we offer home based therapy for clients who are physically unable to attend the centre, or for clients whose rehabilitation goals are predominantly home based. This will be discussed when your first appointment is booked over the phone.

We can suggest alternate cost-effective transport options. Alternate transport options can be discussed when your first appointment is made.

On completion of the program, all participants are provided with a discharge/care plan. Depending on your individual needs, this may include a referral into local community services or an on-going independent program.

Yes, hydrotherapy, group exercise and education programs are available depending on your individual needs and goals. This will be discussed with your primary therapist.
