Temporary Relocation of Services – Upper Yarra Family Centre
Eastern Health has been advised by Yarra Ranges Council that necessary roof repairs are required at the Upper Yarra Family Centre (UYFC). To allow these works to take place, our services will need to temporarily relocate.
Community Health supports the health and wellbeing of people in our community. We provide services that help improve physical, social and emotional wellbeing, particularly for people who may face challenges accessing healthcare or who are at greater risk of poor health outcomes.
We provide services for children, adults and families, as well as specialised support for people living with chronic health conditions and other priority population groups.
Our services are available across several Eastern Health locations, making it easier to access the care and support you need.

Fees and funding
Many Community Health services are funded by the Commonwealth and Victorian Governments through Community Health and HACC programs.

Commonwealth Home Support Program services
The Commonwealth Home Support Program services are provided by our Community Health, District Nursing and Aboriginal Health Teams to improve the physical, social and emotional wellbeing of our community.
