Eastern Health Aged Care Assessment helps older people living in Melbourne’s eastern region access the right aged care supports at the right time. We work with you, your family and carers to understand your needs, goals and circumstances, and to connect you with services that support your independence, safety and wellbeing.
Our service is part of My Aged Care and provides aged care assessments for people who may need government‑subsidised aged care services. These assessments help determine eligibility for services such as:
- Commonwealth Home Support Program
- Support at Home
- Aged care homes (Short-term respite or ongoing care)
- Transition Care
Your aged care assessment is conducted by a qualified health professional as part of a multidisciplinary assessment team, which may include nurses, social workers or allied health clinicians. The assessment is a conversation‑based process that focuses on understanding your health, daily activities, supports, goals and what matters most to you. The assessor will talk with you (and your family or carer if you choose), ask questions about how you manage at home, and discuss options that may support your independence, safety and wellbeing now and into the future.
You may be referred to our service if you:
- Have difficulties managing daily activities due to ageing, illness, frailty or disability, such as personal care, mobility, memory, safety at home or managing health needs, and
- Are:
- 65 years or older, or
- 50 years or older if you are an Aboriginal or Torres Strait Islander person, or
- 50 years or older and homeless or at risk of homelessness.
Eligibility is not based on age alone. Referrals are made through My Aged Care, which completes an initial screening to understand your situation.
- People who are not medically stable
- People who do not consent for an aged care assessment
- Under 65 years of age, unless
- 50 years or older if you are an Aboriginal or Torres Strait Islander person, or
- 50 years or older and homeless or at risk of homelessness.
If you are under 65 years of age (or under 50 for Aboriginal and Torres Strait Islander people, or under 50 and homeless or at risk of homelessness), you are usually supported through other programs, as aged care services are designed for older people.
Depending on your circumstances, you should contact:
- The National Disability Insurance Scheme (NDIS) (phone 1800 800 110), or
- Home and Community Care Program for Younger People (HACC PYP).
If you have complex support needs, The Ability First Australia System Coordinator Program helps support people under 65 who are not eligible for NDIS or aged care services by helping them navigate, coordinate, and access appropriate supports so they can live safely and well in the community. Community Assessments
For existing Support at Home clients, by your Support at Home provider, via your Case Manager.
For all other clients, access to our service begins with a referral through My Aged Care.
Referrals to My Aged Care can be made by:
- You or your family or carer (Phone 1800 200 422 or apply online)
- Hospital staff or your GP
- Community health or aged care providers
My Aged Care will complete an initial screening to understand your situation. If an assessment is recommended, your referral will be sent to an assessment organisation who will contact you to discuss your referral.
Eastern Health Aged Care Assessment are one of several assessment organisations servicing the eastern metropolitan region. Please note, we are only able to assist clients who have an active My Aged Care request with our service.
Hospital Inpatient Assessments
Hospital inpatient assessments are provided for people who require direct entry from hospital to residential aged care (respite or permanent) or transition care following a hospital stay, with referrals made by the hospital social worker or allied health team.
Hospital patients seeking home support are referred for community assessment on discharge.
