This service provides Aged Care Assessment Service (ACAS) assessment and short term case management for clients at risk of a preventable hospital admission or Emergency Department presentation.
Referrals are prioritised according to need, we can usually respond to urgent referrals as required and do not usually have a waiting.
- Requires Aged Care Assessment Service assessment
- Requires short term case management
- History of multiple hospital admissions/Emergency Department presentations or at risk of a preventable hospital admission /Emergency Department presentation
- Resides in a Commonwealth Government funded residential aged care facility
