Eastern Health Acute Specialist Clinics provide specialist non-admitted care to people in acute health settings.
People are referred to specialist clinics by their general practitioner (GP), specialist or community-based health provider, as well as hospital-based clinicians in emergency departments, inpatient wards and other areas of the hospital.

Eastern Health receives and manage referrals in line with the Department of Health’s Managing referrals to non-admitted specialist services in Victorian public health services policy. Access here for more information.
Step 1 – Referral Acknowledgement
• Referrals for all sites are received by the central Specialist Clinics Referrals and Bookings Management Team
• eReferrals will receive an immediate confirmation in your GP’s clinical software that it has been received
Step 2 – Triage and Clinical Prioritisation
• Referral information is screened against state referral criteria and local health service acceptance criteria
• Patients requiring an urgent consultation will be offered an appointment within 30 calendar days.
• Patients requiring a routine consultation will be seen at the next available non-urgent appointment or placed on a waiting list
Step 3 – Appointment/ Waitlist Notification/ Declined Referral
• Patients will receive either a text or letter notification of their appointment details, or if they have been placed on a waiting list or if their referral has been declined.
• Referring clinicians will receive a letter notification of referral acceptance, or declined notice with information outlining the reason.

*Note: Eastern Health reserves the right, at the discretion of the triaging clinician, to request referral redirection to a closer service if there are other health services who can provide prompt, geographically closer or more appropriate services for the patient.
If your condition changes or deteriorates, we recommend you contact your general practitioner. In the case of an emergency please attend the nearest hospital Emergency Department.

You should not use this form if your enquiry is related to Maternity, Oncology (Cancer Services), Maroondah Breast and Cancer Centre, Allied Health, Neurodiagnostics, Ophthalmology, or Endoscopy.

- Update your details before an Acute Specialist Clinic appointment
- Enquire about the status of your appointment
- Request to reschedule or cancel your booked appointment
- Request to be removed from a waiting list
- Request a copy of your Pathology / Imaging form and / or script
- Respond to a validation SMS or letter
- Request information about a Telehealth appointment
