The Access and Support program provides short-term, individual support to people who need help to access Home and Community Care (HACC) services.
The program aims to support people who have trouble accessing services due to barriers such as:
- Memory loss or dementia
- Culturally and linguistically diverse
- Aboriginal and Torres Strait Islander
- Living in remote or isolated areas
- People experiencing financial disadvantage and/or homelessness
Our Access and Support program will actively engage with individuals to assess, plan and support them to link into services that will help them to live independently.
