Quality Care Together (QCT) is the flagship transformational change program that will underpin the Healthier together strategic priorities for Eastern Health.
QCT overarching approach is to enable:
- Standardised quality care and safe care from start to end.
- Organisational trust and empowered roles and responsibilities.
- Remove silos and focus on the patient journey – patient experience and patient partnership in care is key.
- Co-designed local improvements, with success being implemented and executed across the organisation – to deliver quality care together for our consumers and be healthier together with our workforce.
- Embed access and hospital system flow.
