Pharmacist Shared Medicines List (PSML) upload to My Health Record (MHR)

Pharmacist Shared Medicines List (PSML) is a new clinical document type supported by the My Health Record (MHR) system.

It contains detailed information about all medicines the consumer is known to be taking at the time the list is created (i.e includes prescription, non-prescription, OTC medicines and complementary medicines).

Eastern Health will be uploading the IMAC (except ED IMAC simple additions) and PFML that are currently generated using EMR onto MHR in the first week of April; once an IMAC/PFML is signed the upload process will be triggered. The document will only be uploaded if the consumer has consented in the MHR system.

Staff will be able to access the PSML generated by Eastern Health at the point of discharge for patients. There will be no change to end user workflow.

Implementing this means that external care providers will be able to access the IMAC/PFML generated by Eastern Health clinicians via MHR, therefore improving information sharing between health professionals, especially at transitions of care.
