The Continuing Care Program is a multidisciplinary team providing case management for clients with a severe mental illness. This includes people with schizophrenia and other psychotic disorders, mood disorders, severe anxiety disorders and personality disorders (generally borderline personality disorders) and dual diagnosis.
The teams at Murnong Continuing Care Team (CCT), Chandler House CCT and Lilydale CCT are made up of:
- Psychiatric Nurses
- Psychologists
- Social Workers
- Occupational Therapists
- Consultant Psychiatrists
- Psychiatric Registrars/medical Officers
- Administration Staff
Case management is aimed at providing non-urgent intervention for mentally ill clients.
Case management should be considered when the needs of the client cannot be met by other more generic services and where assertive follow up may be required. Referrals are accepted from Outer East triage or other public area mental health service where the client is already case managed.
General Practitioners, non-government organisations, psychiatric disability support services, family members, friends or any one else may refer clients to the triage service, discussing any concerns with the clinician as appropriate.
