The Continuing Care Program is a multidisciplinary team providing case management for clients with a severe mental illness aged 18* to 64 (*Clients aged between 16 and 25 may be seen by either the child, youth and family services or continuing care services).
This includes people with schizophrenia and other psychotic disorders, mood disorders, severe anxiety and personality disorders (in general the borderline personality disorders) and dual diagnosis.
The teams at Koonung Continuing Care Team (CCT), Doncaster CCT and Waverley CCT are made up of:
- Psychiatric Nurses
- Psychologists
- Social Workers
- Occupational Therapists
- Consultant Psychiatrists
- Psychiatric Registrars/medical Officers
- Administration Staff
Case management is aimed at providing non-urgent intervention for mentally ill clients.
Case management should be considered when client needs are not met by other more generic services and where assertive follow up may be required. Referrals are accepted from Eastern Health Psychiatric triage or other public mental health service where the client is already case-managed.
General practitioners, private providers, psychiatric disability support services, friends or family members can refer clients to the triage service to discuss concerns with the clinicians.
The catchment area for Koonung CCT is the city of Whitehorse (west of Springvale Road), Waverley clinic serves the northern section of the city of Monash (north of Dandenong Road). The boundary between these two Teams is Highbury Road. Doncaster CCT services the city of Manningham.
