Eastern Health Diabetes Education comprises a team of highly qualified nurses who provide personalised diabetes education and care to both inpatients and outpatients of Eastern Health hospitals. In 2016, we were pleased to deliver 13,400 consultations to people with diabetes and other health professionals.
You can be referred to the Diabetes Education service if you.
- Are a current inpatient at Eastern Health
- Are currently attending an Eastern Health Endocrinology and Diabetes outpatient clinic
- Are at risk of diabetes, have pre-diabetes or diagnosed with diabetes can be seen by Eastern Health Community Health at Healesville Hospital & Yarra Valley Health. You can self-refer or the GP can refer for Diabetes Education.

Diabetes Education provides the following:
- Help you and your family manage diabetes with confidence, skill and motivation through:
- Understanding a diagnosis of diabetes
- Gestational diabetes services to support women with diabetes in pregnancy (not at Healesville Hospital & Yarra Valley Health)
- Self blood glucose monitoring, including the use of “Smart” meters
- Registration with National Diabetes Services Scheme
- Understanding the role of oral medications and insulin in diabetes management and use of insulin devices
- Continuous glucose monitoring (CGM) and Insulin Pumps
- Strategies to deal with recurrent or severe episodes of hypoglycaemia
- Sick day management - Provide nursing, medical and allied health staff with diabetes education and resources e.g. development, delivery and evaluation of an innovative education program called ‘Diabetes – Topics in a Flash’ to nursing staff
