Palliative Care Inpatient Unit

Palliative Care Inpatient Referral Form

The Wantirna Health palliative care inpatient unit is a 32 bed specialist palliative care unit. We provide care for patients with life limiting illnesses. People are admitted for various reasons, including symptom control, end of life care, respite, and assessment. The unit is not able to offer long term care.  Some patients come to us to die however others are able to return home with support.

Frequently asked questions

Fill out the referral form and send it to our Continuing Care Access unit at Peter James Centre

Yes, the palliative care team could discuss goals of care with your father, improve his breathlessness, organise help and support at home, and make plans for his future care that could help avoid being admitted to hospital.

Whilst you are still capable of discussing your healthcare with your doctors, your advance care plan will not be used. It is only used when you are unable to communicate your own decisions or lose the ability to make decisions due to dementia or some other condition.

Most of our rooms are single rooms however we do have some double rooms. We cannot guarantee you a single room as these are allocated to patients on the basis of need.
