The Transition Care Program (TCP) is a Commonwealth and State funded program. The Eastern Health Transition Care Program provides short term support and care to older people for up to twelve weeks at the end their hospital stay.
As a goals based program, TCP can be delivered in the home (home based) or in a residential care setting (bed based). It is also possible to transfer from one care setting to another. The program is designed to assist older people who can achieve further improvements in function with the plan of continuing to live independently, or, alternatively, assisting them while their family and carers make appropriate long-term care arrangements. When admitted to the program, the allocated TCP case manager will work closely with you to coordinate your individual care needs and future care planning.
The Eastern Health TCP have seventy-two residential/bed based packages located at two residential care sites within our region and twenty-seven community/home based packages.

Referral
Enquiries can be made directly to the Eastern Health TCP Administration team who will be able to advise as to the referral process. The Administration team are available Monday to Friday, during normal office hours (i.e. 9.00 a.m. and 5.00 p.m.) via phone on (03) 9955 7585 or by email: Transition.Care@easternhealth.org.auFees
TCP Client Fees are adjusted by the Commonwealth twice yearly (March and September) in alignment with changes to the Age pension.- 17.5 per cent of the basic, single aged care pension for the home based service
- 85 per cent of the basic, single aged care pension for the bed based service
