Services

Transition Care Program (TCP)

Transition Care aims to help older people complete their recovery following a hospital stay. The program can be provided in the community or in a residential setting. Each client will receive an individualised care plan and their length of time on the program will vary depending on these individual needs and goals.

The services provided as part of TCP will vary, ranging from services which improve a care recipient’s capacity for independent living to services that enable a care recipient to have the time to plan and to enter residential aged care at an optimal level of functioning.

Eligibility Criteria

Clients must be medically stable, aged 65 years or over (or with aged related disabilities) and be a current hospital inpatient. The Aged Care Assessment Service (ACAS) assess and approve the eligibility of clients for the Transition Care Program in accordance with the Aged Care Act and the ACAP Program Guidelines.

Cost

The cost is calculated at 85% of the current single aged pension for residential care and 17.5% of the current single aged pension for community based TCP.

TCP Administration: Mon – Fri 9.00am – 5.30pm

Visiting Times : There are no fixed visiting times for the TCP residential sites. If you have a query, please contact the facility manager or, when admitted, with the TCP case manager.
