What is Connect@Home?

Connect@Home is a short-term Eastern Health service that helps you settle safely at home after a hospital stay. Our multidisciplinary team – a doctor, a nurse or a pharmacist – works with you by phone or telehealth in the first few days after you leave hospital to make sure you have the support, information and connections you need.

We are not a long-term service. We work with you for a short period and then hand over to your GP, to other Eastern Health’s Care@Home services and to any ongoing community supports you need.

Connect@Home supports adults who:

  • Are returning home from an Eastern Health hospital or service.
  • Would benefit from short-term multidisciplinary telehealth support to stay well at home.
  • Need help coordinating follow-up, medicines review, or linking with community services.

Your hospital or community team will refer you to us. You do not need to refer yourself.

Within 24–72 hours of going home 

  • One of our team will contact you — usually by phone.
  • We will ask how you are, check how you are managing, and ask about your medicines.
  • We will talk with you about any concerns, help you understand what your hospital team has recommended and agree what to do next.
  • We will help you connect with other services if you need them — for example, the Hospital Admission Risk Program (HARP), My Aged Care, Carer Gateway, Allied Health and community supports.

Ending with Connect@Home 

  • We work with you for a short time.
  • When we finish, we make sure your GP has all the information they need, and that any ongoing services are in place when required.
  • If your situation changes later, your GP or hospital team can refer you back to support and help navigate your care.

Connect@Home is part of Eastern Health’s wider network of community and home-based services. Depending on what you need, we can help you connect with:

  • Hospital Admission Risk Program (HARP)
  • Care@Home services — including Hospital in the Home (HITH), Rehabilitation at Home, Geriatric Evaluation and Management at Home (GEM@Home), Transition Care Program (TCP), and Early Stroke Discharge.
  • Community Rehabilitation Programs (CRP) and Community Rehabilitation Teams (CRT).
  • Post Acute Care and Subacute Community Services (SACS).
  • Residential Inreach for people in residential aged care.
  • Yarra Valley District Nursing Service (YVDNS).
  • Acute Specialist Clinics.
  • Your GP, community pharmacy and any specialists you see.

Other 

Who will I hear from?

A nurse, doctor or pharmacist from the Connect@Home team. They will introduce themselves and explain what they are calling about. Calls are usually from an Eastern Health number.

Do I have to pay?

No. Connect@Home is a free public hospital service for eligible clients.

What if I miss the call?

We will try again. You can also call us back on 9955 7799. If you are unwell and cannot wait, please contact your GP, Nurse-on-Call, or in an emergency call 000.

How long will I be with Connect@Home?

Most clients receive one telehealth follow up. Further follow up and support may be provided. We will agree with you when we finish and make sure your GP is updated.

Can my family or carer be involved?

Yes. We encourage this. Let us know who you would like us to speak with and we will do our best to involve them.

What if I need an interpreter?

Tell us what language you prefer and we will arrange an interpreter. There is no cost to you.

What if something is wrong out of hours?

Outside our operating hours, contact the Victorian Virtual Emergency Department, call Nurse-on-Call on 1300 60 60 24, or contact your GP.

I used to hear this called Talk To You Tomorrow — is it the same service?

Yes. Connect@Home is the new name for the Talk To You Tomorrow program. The team, the service and the phone number have not changed.
