Commonwealth Home Support Program services are provided by our Community Health, District Nursing and Aboriginal Health Teams to improve the physical, social and emotional wellbeing of our community.

National Aged Care Advocacy Program:

NACAP supports older people who receive or are applying for government-subsidised aged care by:

  • providing advocacy services
  • helping them to understand and access the aged care system
  • informing them of their aged care rights

Aboriginal Health

The Aboriginal Health Team is a community-based health service located in Healesville available for Aboriginal and Torres Strait Islander people and their families living anywhere in Eastern Health’s catchment. The service adopts a holistic approach in line with cultural best-practice, and provides a broad range of services which are available both on-site in Healesville and through home visits and outreach.

Commonwealth Home Support Program Services include:

  • Domestic assistance and shopping assistance
  • Yard Maintenance
  • Social Support – Group

For eligibility for these services, you will need to have a My Aged Care Assessment.
