Eastern Health Residential Aged Care Services (RACS) comprises two homes; murrenda Aged Care Home located in Wantirna and Monda Lodge Hostel located in Healesville.
Eastern Health Transition Care Program provides short term support and care to older people for up to twelve weeks at the end their hospital stay and is delivered in the home or in a residential care setting
Eastern Health Commonwealth Home Support Program (CHSP) services are based from the Healesville Hospital and Yarra Valley Health site.
CHSP services are provided within Community Health and Aboriginal Health services.
