Eastern Health representatives joined the Chair of Community Health First, Anna Robinson in welcoming The Minister for Health, The Hon, Harriet Shing and Member for Monbulk, Daniela De Martino to Each in Ferntree Gully to mark recent investment by the Victorian Government in supporting people with chronic conditions to stay well at home.
The Victorian Government has committed more than $20 million over four years to expand the Care Pathways for Chronic Disease program, building on Health@Home pathways delivered in the collaboration between Eastern Health and Each.
Alongside Andrea McDougall, Manager, Clinical and Complex Care at Each, Eastern Health Director Operations, Care@Home & Acute Specialist Clinics, James Campbell, is excited by the funding announcement and looking forward to the expansion of the project.
“We are proud to have pioneered this approach with Each and look forward to supporting the expansion of the initiative across more community health services.”
“The program provides tailored, longer-term support to help people manage their health at home, reducing avoidable hospital presentations and improving quality of life.
For every patient who avoids an unnecessary hospital admission, there is a ripple effect, less pressure on emergency departments, more capacity for acute care, and a stronger health system overall” he said.
The event also featured a speech from John Terribile, a consumer currently receiving care through the program. He shared personal insights about the value of staying connected and how his support has helped him in his recovery journey.
Around 4,500 Victorians are expected to benefit as the program expands across community health services statewide.
“This is a critical step towards better preventative care. Care Pathways for Chronic Disease will help people better manage their condition, deliver stronger health outcomes for our communities, and ease the burden on hospitals”, said Anna Robinson.
Eastern Health looks forward to continuing this work with partners across Victoria to deliver care closer to home.
