Emergency care in the home

The Falls Outreach Risk Mobile Assessment Team (FORMAT) aims to assess residents of Aged Care facilities on-site, bringing the emergency room to the most vulnerable people in our community.

During the FORMAT pilot period, the emergency department saw a 28% reduction in residents presenting after they had experienced a fall.
