The Older Adult Mental Health Community Service provides community-based treatment, care and support for older people with severe mental illness. The team partners with consumers, their families/carers and their General Practitioner and other community service providers to identify and address their needs, and assist in recovery through:
- Comprehensive mental health assessment
- Tailored treatment and therapy
- Linkage to other services
- Advocacy
- Group therapy
- Liaison and planning to establish ongoing community supports
- Shared care with General Practitioners and Community Support Services.
When a referral is accepted for a mental health assessment, the person or their next of kin will be contacted by a mental health clinician.
An assessment will usually be conducted by a mental health clinician and doctor to assess the needs and supports required by the person and family/carer.
Following the assessment, and in collaboration with the person, their family/carer and their general practitioner, a recommendation will be made for appropriate treatment or service.
Wellness in the Home is a specialised service providing seven day per week mental health care in a person’s home setting. This team supports people requiring more intensive support in a community setting.
