Make your wishes about your mental health care known to us in case a situation arises where you are unable to communicate what you want. This statement helps to make sure that your voice guides your treatment and care throughout your journey, including situations where you may be subject to compulsory assessment or treatment.

If you wish to withdraw your Advance Statement of Preferences you may do so by completing and submitting this form:

If you are currently engaged with one of our mental health & wellbeing services, please submit your statement directly to your treating team.

Otherwise, Advance Statement of Preferences can be submitted via the email address: AdvanceStatements@easternhealth.org.au. If you have not previously been a patient at Eastern Health, we will need to open a medical record on your behalf. An Eastern Health staff member will contact you to discuss this with you.

We also recommend uploading to your My Health Record so your plan is visible when you attend a healthcare setting across the country

If you want to begin writing an Advance Statement of Preferences, use the simple way.

Ask yourself,

What are things that you like? Some things that you do to be happy or be calm or you enjoy.

What are things that you don’t like? Some things that don’t help or trigger unhelpful things.

What are some recovery ideas? Some things that help you be well.

You can then put this on the fridge. And you can go to this anytime you need.

If you prefer not to put it on the fridge, you can put it next to your bed.

The idea behind this is, in the next few months you might develop a more detailed Advance Statement of Preferences.
