The Ambulatory and Community Services provide nursing, medical, allied health and community health services that deliver health care to support patients and clients in their transition from hospital to home or to receive care in a clinic or at home.

The program aims to create opportunities and supportive environments which empower people to develop healthy lifestyles and prevent or manage illness.
