Your Post‑Discharge Medicines Information

Accurate and easy‑to‑understand information about your medicines after leaving hospital 

When you leave hospital, it’s important that you, your General Practitioner (GP), and any other healthcare providers have the most accurate and up‑to‑date list of your medications. To support your safe discharge from hospital, the team will upload your medication information to My Health Record within 24 hours of discharge. 

Below are the key documents that may be created for you on discharge.  

Patient Friendly Medication List (PFML)

This is the most helpful and complete list of your medicines after you leave hospital. 

What it includes 

  • Every medication you take 
  • Clear explanations of any changes made in hospital 
  • Simple instructions on when and how to take you medications  

Why it’s important 

  • Helps you understand your medicines 
  • Supports safe follow‑up with your GP or other health providers 

Access 

  • You will usually be given a printed copy to take home by a pharmacist or doctor on discharge 
  • Your GP can also view the PFML in your My Health Record, in case you forget your copy. 

Other Documents You May See in My Health Record

This is a general summary of your hospital stay which will be given to you on discharge and/or accessible on My Health Record. 
It includes: 

  • Diagnoses 
  • Treatments 
  • Tests and results 
  • Follow‑up instructions 
  • A summary of medication changes 

Important: 
While the discharge summary is helpful, your PFML or IMAC (if you received one) contains the most complete and pharmacist‑checked medicines information. 

These should be used first when checking your current medications. 

You can access your information at any time online. 

  1. Log into My Health Record through myGov. 
  2. Select Documents 
  3. Select Clinical Records 
  4. Look under:  
    1. Pharmacist Shared Medications List – for your PFML or IMAC 
    2. Discharge Summaries– for your Discharge Summary 

If you have trouble accessing My Health Record, your GP or local community pharmacist can access these documents using their clinical software (with your permission).
