Accurate Comprehensive Post‑Discharge Medicines Information via My Health Record (MHR)
Most Accurate, Comprehensive Discharge Medication Documents
Patient Friendly Medication List (PFML) and Interim Medication Administration Chart (IMAC) contain the most current and reliable information. They are prioritised for eligible patients and uploaded to My Health Record (MHR) within 24 hours of discharge.
1. Patient Friendly Medication List (PFML)
- Clear, patient‑centred list including all medication changes.
- Printed copy supplied to eligible patients to take to their GP appointment.
- GPs can access the PFML on MHR if the patient does not bring their copy.
2. Interim Medication Administration Chart (IMAC)
- A temporary medication chart to support safe continuation of mediation administration immediately post‑discharge.
- Provided to patients being discharged to Residential Aged Care Facilities (RACFs) or Supported Residential Services (SRSs).
- GPs can access the IMAC on My Health Record if the receiving facility is unable to provide an IMAC.
Other Documents in My Health Record
A summary of the hospital admission outlining the relevant diagnoses, treatments, medication changes and follow‑up requirements. Medication information in hospital Discharge Summaries provides important clinical context, however, PFML and IMAC offer more comprehensive, pharmacist‑verified medicines information at discharge and should be used as the primary reference when available.
- Open the patient in your My Health Record enabled clinical software
- Navigate to My Health Record
- Select Clinical Documents
- Select Medicines View
- Review Pharmacist Shared Medication Lists (PFML or IMAC if available) and the discharge Summary
