I grant to Eastern Health the right to make a recording of my image, voice, identifying material or other information about me for audio or visual reproduction (Recording).
I agree the Eastern Health will own all rights in the Recording. To the extent that I own any rights in the Recording, I assign all present and future rights (including intellectual property rights) in the Recording to Eastern Health.
I understand that the Recording may be edited or adapted at Eastern Health’s sole discretion.
I agree that Eastern Health may, both now and in the future, use and authorise the use of the Recording in while or part in any manner and in any media that Eastern Health considers contextually appropriate,
This may include for promotional, educational or information purposes.
The Recording may be used in brochures, newsletter, annual reports, magazines, local/national media, television and social media, which may be accessed both in Australia and overseas. I acknowledge and accept that I am not entitled to any payment for the Recording and I release and indemnify Eastern Health in relation to any loss, damage, cost expense or claim arising out of Eastern Health’s use of the Recording, including defamation, libellous material, breach of privacy or copyright claims.
Eastern Health will endeavour not to attribute the image in a way that may adversely impact the person granting consent. This includes, not using the image for a purpose that may detrimentally impact the individual in the content.
The intent is not to provide the content for any purpose outside of Eastern Health.
