Requests for the release of information pertaining to the Family Violence Information Sharing Scheme or Child Information Sharing Scheme must be completed by an Information Sharing Entity (ISE) and/or Risk Assessment Entity (RAE).
To request information, or to voluntarily share information under these schemes please email our Information Release Service (inforelease@easternhealth.org.au) with the name and date of birth of the patient. Our Information Release Service will arrange for an Eastern Health authorised person to contact you and facilitate the request.
