Referral Criteria
Referrals to this service are assessed against the Statewide Referral Criteria and will be declined if the criteria are not met. For all other Vascular conditions, please continue to refer as per clinical assessment and need.
- Aortic aneurysm
- Carotid artery disease
- Deep vein thrombosis
- High-risk foot ulcers
- Hyperhidrosis
- Lymphoedema
- Non-healing or chronic lower leg ulcers
- Peripheral Arterial Disease
- Refer to Eastern Health “Service-Specific Criteria” in Health Pathways Melbourne (Urgent or Routine Vascular Surgery Referral).
- Varicose veins
- Refer to Eastern Health “Service-Specific Criteria” in HealthPathways Melbourne (Urgent or Routine Vascular Surgery Referral).

Clinic for the entire range of arterial and vascular disease, especially aneurysms, carotid disease and lower limb ischaemia.
Referral
GPs are encouraged to used the Victorian Statewide referral Form (VSRF) which can be downloaded into most medical software.
Please send results of any relevant tests, Doppler studies and ultrasounds. Imaging films or CDs should be bought by the patient to the clinic appointment.

Bulk Billed - no cost to patient
