Yarra Valley District nurses provide nursing care to people in their own homes, every day of the year.
Our District Nursing Service is supported by funding from the Commonwealth Home Support Program, Department of Veterans Affairs and Victorian Governments including Home And Community Care Young Persons Program and Post Acute Care.
Examples of care that can be provided:
• People who need care immediately following their discharge from hospital
• End of Life and /palliative care
• People of any age who have a disability and who need nursing assistance to manage their healthcare needs
• Wound Care
• Diabetes management
• Continence management
• Medication managementHelping you find and link with other community services for older people.
