Community Rehabilitation Program

“Rehabilitation is a process which enables people to reach and maintain their optimal physical, sensory, intellectual, psychological and social performance. Rehabilitation gives people the tools they need to reach their independence…” (WHO, 2015).

The role of Community Rehabilitation Programs is to optimise a person’s functional ability and independence.

Community Rehabilitation offers short term goal directed rehabilitation aimed at improving client’s function and independence following a change in function due to recent acute medical / health event.

This is achieved by developing a care plan based on goals that are negotiated between the client and their team (see below for team details).

Therapy can either be delivered in a group or individual setting. Clients can be seen in their own home or in one of our centres, dependent on their needs and goals.

Eligibility Criteria

  • Resides in Eastern Health catchment.
  • Client needs allied health input and can be managed medically by GP (nursing care is not available in this program).
  • Requires short term goal directed subacute rehabilitation.
  • Has a change in function / activity limitation due to recent acute medical / health event.
  • Clients are required to have functional rehabilitation goals.
  • Client consents to participate.
  • Adults over 18 years.

Referral

Doctor or allied health referral preferred, however self-referral accepted

Cost

There is a cost for our service, which we will explain to you when we call to discuss your treatment plan.

We understand that covering the cost of health can be hard, even if you received the service in a public hospital. If you are worried that you may not be able to pay, please contact Patient Revenue Services to talk through your situation.
