Referral Form

This service provides speech pathology for babies and pre-school children with communication and feeding difficulties. Reasons for referral may include:
- Difficulty/delay in using words or forming sentences
- Difficulty/delay in understanding the meaning of words and sentences
- Difficulty/delay in using speech sounds (articulation)
- Stuttering
- Difficulties with feeding (i.e. fussy/restrictive eating, difficulties transitioning to solids). Please see our Paediatric Feeding Clinic page for more information..
Therapy is provided to children and families individually or in small groups. Appointments are conducted either face-to-face or using telehealth.
Referral
Please note: Referrals for children starting school the next year must be received before the 30th of June.
Parents or carers are able to self-refer a child for assessment. Referrals are also accepted from any professionals involved with the child (such as a Maternal and Child Health Nurse, Kindergarten Teacher), as long as consent from the parent/carer is obtained.

Any investigations e.g. hearing assessment should be forwarded with referral.We are unable to accept referrals for
- Children who qualify for early intervention services (ECIS / ECEI) or NDIS
- School aged children
- Children attending another service
Cost
The level of fees payable is dependent on the client’s income and whether the client has a concession card. Current fees (February 2019) are:
- There is no cost for children from low income families (includes Health Care Card holders),
- $15 for middle income earners
- $100 for high income earners.
