The Continuing Care Program is a multidisciplinary team providing case management for people with a severe mental illness.
Continuing Care staff work with consumers, their carers, families and other community agencies to identify and address their needs, coordinate their care and assist in recovery through:
- Psychiatric, psychological, social, functional and family assessment
- Linkage to other agencies
- Advocacy
- Individual, family and group therapy
- Liaison and planning to establish post-discharge supports and minimise the risk of relapse.
- Crisis response including referral to more intensive support where required
- Shared care with General Practitioners and Psychiatric Disability Rehabilitation & Support Services.
