The Aged Care Assessment Services (ACAS) is an independent team who assists older people and their carers to identify what kind of care will best meet their ongoing needs.

Teams are multi-disciplinary including health professionals such as medical officers, social workers, nurses, occupational therapists, physiotherapists, speech pathologists and dietitians.

ACAS conduct comprehensive assessments for people needing community or residential aged care services including restorative and transitional care options.

ACAS assessment is an eligibility requirement to access Commonwealth funded residential aged care, residential respite care, transition care and home care packages.

The Eastern region ACAS teams provide service to the following Local Government Areas: Manningham; Monash; Whitehorse; Knox; Maroondah; and Yarra Ranges. If you would like to refer outside of these areas please visit the My Aged Care website for referral or phone 1800 200 422.

Eligibility Criteria

Persons aged 65 years and over or 50 years and over if you identify as Aboriginal or Torres Strait Islander.

People under 65 years of age will usually first require an assessment for eligibility for the National Disability Insurance Scheme (NDIS). For further information on the NDIS in your region please contact 1800 800 110 or visit the NDIS website.

Exclusion Criteria

  • People who are not medically stable
  • People who do not provide consent for an ACAS assessment
