The Residential Inreach Service provides an alternative to emergency department presentations for clients in residential aged care facilities (RACF’s). This service aims to support clients and staff to manage acute health issues when GP or locum is unavailable.
Referrals are triaged and followed up by specialist nursing/medical staff as required.
We can provide:
- Medical assessments of acute conditions
- Phone support and advice to facility staff about clinical decisions(our nurse can visit if required)
Acute Conditions can include:
- Chest infection/ UTI/ Cellulitis
- Gastroenteritis
- PEG and urinary catheter trouble shooting (not routine changes)
- Wound complications e.g. infection
- End of Life care and support to facility staff in care management
