District nursing
District nurses provide nursing care to people in their own homes, every day of the year. Services include generalist and specialist nursing.
District nurses provide professional nursing and healthcare in the following areas:
- People who need care immediately following their discharge from hospital/day surgery
- People of any age needing palliative care
- People of any age who have a disability and who need nursing assistance to manage their healthcare needs
- Wound Care - These wounds can include leg ulcers, lacerations and skin tears or wounds that have not healed properly after an operation
- Diabetes management - All our nurses are skilled in diabetes management. We also have nurses who are experts in managing diabetes, and work alongside our general staff to ensure that your diabetes is managed in the best way possible.
- Continence management - We have nurses who are experts in continence management, known as Clinical Nurse Consultants. These specialist nurses may visit to assess your continence and provide advice and support. They also work with our general nursing staff to provide a complete care package.
Aged Care
Our nurses can help you by:
Assessing and managing memory loss
Managing medication
Setting up a safe hygiene routine
Helping you care for a frail, sick or confused older person
Helping you find and link with other community services for older people.
