Transition Care Program (TCP)
TCP is a goal orientated, time limited program for older people who have completed their hospital stay.
TCP is a national program. Eastern Health TCP has four residential sites located with the EH region which provide residential TCP care.
Transition Care is a Commonwealth and State Government funded program which aims to help older people complete their recovery following a hospital stay. The program can be provided in the community or in a residential setting for a period of up to 12 weeks.
To be eligible for Transition Care, the client must be an older person who is hospital inpatient. They need to have completed their acute stay (i.e. be medically stable) and any necessary sub-acute care (rehabilitation). The Aged Care Assessment Service (ACAS) assess eligibility for entry to TCP.
The services provided as part of TCP will vary, ranging from services which improve a care recipient’s capacity for independent living (which would likely involve input from various members of the therapy team) to services that enable a care recipient to have the time to plan and to enter residential aged care at an optimal level of functioning.
Transition Care can also be a useful option for those people who need more time to decide whether return home or moving in to residential care is their preferred choice.
The TCP case manager works closely with client (and families as directed), in order to achieve a positive outcome.
