Eastern Health hospital admission risk services are based across Box Hill, Maroondah and Angliss Hospitals as well as many community facilities in the eastern region.
We provide a service that coordinates health and community services with you. This involves your GP, medical specialists, hospital staff, community services and your carer/s.
It is about providing the right medical and community services for you to maintain your health.
