ACAS
is part of the Commonwealth Government's Aged Care Program and one
of 18 teams in Victoria and 120 teams nationally.
ACAS
aims to provide the community (including its health care facilities
and other service providers) with access to a supportive and educational
assessment and referral services for older people (generally 70yrs
and over) and some younger disabled people and their carers, to facilitate
informed choice of lifestyle either at home or in a residential setting,
and to advocate and help sustain that choice.
The
ACAS teams are multi-disciplinary and include geriatricians, community
health nurses, physiotherapists, occupational therapists and social
workers. The teams work closely with and refer to the Aged Persons Mental Health
Assessment and Treatment Service in both locations, as well as many
other aged care and dementia specialist services and aged care programs,
both within health care facilities, residential aged care facilities
and in the community across the Eastern Metropolitan Region of Melbourne
