YARRA VALLEY COMMUNITY HEALTH
YVCH is a member of Eastern Health and provides primary care services to the Yarra Valley communities across the Shire of Yarra Ranges.
The Yarra Valley Community Health Service strives for a healthier Yarra Valley by:

- Taking steps to prevent disease promote wellness and improve quality of life
- Strive for the delivery of those services and supports in an equitable manner.
- Providing a holistic individualised program of care, regardless of creed, nationality or financial status.
- Ensuring quality of care is client-focused through appropriate resources, to enhance desired outcomes.
- Providing equity of access and separation through a seamless co-ordinated approach of care, to meet individual client’s needs.
Services at YVCHS include:
Advocacy, Antenatal care (Healthy Mothers, Healthy Babies) Asthma Education, Breastfeeding Clinic , Breast Screening, Children’s Services, Carer & Social Support, Community Development, Counselling, Diabetes Education, Dietetics, District Nursing, Early Intervention Program for Chronic Disease, Grief & Loss Counselling, Health Promotion & Educational literature, Co-located GP clinic, Indigenous Health, Needle & Syringe Program, Occupational Therapy, Physiotherapy, Paediatric services, Podiatry, QUIT Courses, Social Support Groups, Speech Pathology, Women's Health Clinics, Youth Resources
YVCH has a single point of entry intake and referral service co-ordination unit five days a week.
The Service Coordinators can provide information and referral to other services including:
Alcohol and other Drug agencies, Family Violence services, Legal Services, Material aid
Mental Health Services, Parenting courses, Social support services, ECASA Sexual Assault Services, Volunteer transport.
