Yarra Ranges Palliative Care Clinic

Yarra Ranges Health Palliative Care Clinic is a multidisciplinary service that provides specialised assessment and support for people needing ‘early’ palliative care interventions, as well as a specialist medical clinic for people with more advanced disease.  The service works in collaboration with your existing health care team such as General Practitioner, medical specialist and community nursing and palliative care service.

The multidisciplinary team consists of palliative care specialist doctor, clinical nurse consultant (manager), occupational therapist, social worker and music therapist.  If your needs are not covered by this group we will make a referral to an appropriate service to assist – such as physiotherapy or dietetics.

Catchment area is unlimited however this depends on how far the patient is willing to travel to an appointment.

How do I decide which palliative care service to refer to?

If the patient is too unwell to attend an appointment outside of the home then choose a home based palliative care service.

If the patient is well enough to attend an appointment and this does not create burden then they are appropriate to attend YRHPCC.


What is the difference between YRH palliative care clinic and Eastern Palliative Care (EPC)?

YRHPCC and EPC are two separate organisations but we do share clients and communicate between services to meet the needs of people.
The focus of YRHPCC is early palliative care.
EPC is a community based palliative care service, they make visits to the home.
