Eastern Metropolitan Region, Cardiac Rehabilitation Services Working Group: Final Report.
The attached report available in PDF format (Large *PDF file: 4333 Kb) outlines the findings of the multi-sector working group focussing on the needs and gaps for cardiac rehabilitation across the Eastern Metropolitan Region during the 2006 and 2007 years.
The report includes a broad range of information that was collected to assist the working group with their review and to identify recommendations for future service delivery planning and to improve the health outcomes for clients living with cardiac disease.

Throughout the life of the project a close liaison was maintained with the Outer East Care in Your Community (CIYC) pilot project with sharing of data and information to support about cardiac disease occurring both ways. The executive summary of the CIYC, Outer East Pilot 06-07, has been included in the Appendices.
