Eastern Health receives a funding grant for Health Promotion through the Department of Human Services, Victoria, Community and Women’s Health funding program. This particular grant is shared across the following Eastern Health sites: Angliss, Box Hill, Maroondah and Yarra Valley Community Health Service. It is targeted at health promotion interventions that meet with current state and regional population health planning and priorities, evidence based practice, ongoing evaluation and strengthening partnerships between service providers to improve population health across the long term.
The Health Promotion Plan has been developed by Eastern Health staff in consultation with service providers and consumers with an overall vision for the following three years of work.

- Part Two: Site focussed Operational Templates for each site for the 2006-07 period.
