Hospital Admission Risk Program (HARP)
HARP provides service coordination and case management of clients who have specific chronic medical conditions and/or complex needs with the aim of reducing avoidable hospital admissions.
Examples are: Chronic Heart Failure, Chronic Respiratory Disease and Diabetes. This also includes Complex Psychosocial support with issues such as drug and alcohol, homelessness, acquired brain injury, personality or behavioural issues, depression and/or anxiety .
HARP operates across Eastern Health, providing service to all sites as needed.
Eligibility Criteria
- Clients must reside in the Eastern Health catchment
- Have had at least two emergency department presentations in the last twelve months or at risk of unplanned/preventable admission
Referral
Exclusion criteria:
- Case managed by another services
- Risk factors which preclude home visits
- Could be more effectively assisted by other services, e.g. Area Mental Health services for acute mental health issues.

HoursCurrently the service operates during business hours, Monday to Friday. From 2012, service will be operating seven days per week.
