Transition Care Program (TCP)
TCP is a goal orientated, time limited program for older people who have completed their hospital stay.
TCP is a national program. Eastern Health TCP has four residential sites located with the EH region which provide residential TCP care.
Eligibility Criteria
Clients must be medically stable, aged 65 years or over (or with aged related disabilities) and be a current hospital inpatient. The Aged Care Assessment Service (ACAS) assess and approve the eligibility of clients for the Transition Care Program in accordance with the Aged Care Act and the ACAP Program Guidelines.
Cost: The cost of TCP is in accordance with the TCP Guidelines and is calculated at 85% of the current single aged pension for residential care and 17.5% of the current single aged pension for community based TCP.

Enquiries can be made directly to the TCP Administration Team who will be able to provide the Eastern Health TCP Referral Form.

HoursTCP Administration: Mon – Fri 8.30 a.m. – 5.00 p.m. Visiting Times : There are no fixed visiting times for the TCP residential sites. If you have a query, contact the facility manager or, when admitted, with the TCP case manager.
