Eastern @ Home
Eastern @ Home provides a Hospital in the Home service which is care equivalent to admission to a hospital bed. Patients remain under the responsibility of a medical/surgical unit and service occurs in the patients home or residential facility, including Nursing Homes and Hostels
Eastern @ Home also provides a program that aims to assist clients and staff in residential settings to access nursing and medical assessment in their own environment for acute health issues. This can be an alternative to presenting to an Emergency Department. This service is available 7 days a week from 8 am - 4.30 pm for phone consultations/advice, visits and assessment.
Eligibility Criteria
Medically stable
Patient lives in Eastern Health catchment area
No history of intravenous drug use
Environment safe for nursing staff (no history drugs, violence, dogs, access difficulties
Referral
Doctors: GPs who are credentialled to this service

For residential inreach services referrals can include
Nursing issues such as blocked PEG Tubes/Supra pubic catheters, Indwelling urinary catheters and wound issues
Medical issues: acute on chronic illness management, urinary tract infections, electrolyte imbalances/dehydration requiring Intravenous antibiotic
